How hyaluronic acid works in the skin — why it hydrates sometimes and builds volume at other times

Few ingredients in cosmetology are as recognizable — and as confusingly understood — as hyaluronic acid. We hear about it from the advertisement of a “deeply hydrating" cream and from the aesthetic medicine office, where the same word describes the modeling of the cheek or lips. And here arises justified surprise: how is it possible that the same ingredient can either hydrate the skin with water or lift the entire area and give it shape? The answer is surprisingly simple, though rarely stated directly: it is not “the same" hyaluronic acid. The molecule is chemically the same — but the form in which it reaches the skin can be drastically different. Sometimes it is liquid and free like water, sometimes dense and elastic like jelly, which can support the weight of tissue above it. The whole secret of the “many faces" of hyaluronic acid lies in one technological procedure — and in one extraordinary property of this molecule. In this text, we break it down into its components: what exactly is hyaluronic acid, why does it absorb water so well, what makes one preparation “dissolve" while another “holds its shape," and why this ingredient has a feature that almost no other in aesthetic medicine possesses — reversibility.

What is it at all — and why does the body produce it on its own

Let’s start with a fact that surprises many: hyaluronic acid is not foreign to the body. It is a natural building block of our skin — a substance that we produce ourselves and that fills the space between cells, acting as a kind of gel-like “filler" and water reservoir. It is in the skin, in the joints, in the eyeball. When you hear “hyaluronic acid," don’t think of something artificial injected into the body — think of replenishing what has been lost over time.

And here lies the crux of the aging problem: as the years go by, the skin produces it less and less. This is one of the silent reasons why mature skin becomes thinner, less firm, and more “thirsty" — its natural reservoir of moisture and fullness shrinks. Treatments with hyaluronic acid are essentially an attempt to give the skin back what it once had in abundance.

💡 One molecule can hold an astonishing amount of water. Hyaluronic acid belongs to molecules that bind water in an unimaginable way — a single molecule can hold multiple times its own weight in water. That’s why it is called a “molecular sponge." It’s not about “adding water" like pouring into a glass — it holds it and prevents it from evaporating. Hence the characteristic feeling of tight, filled skin from within.

First role: a sponge for water (i.e., hydration)

The simplest, gentlest version of hyaluronic acid is its hygroscopic role — that is, the ability to attract and bind water. In this form, the molecules are small, free, and not connected in a rigid network. Injected shallowly into the skin (or massaged into its surface in a cosmetic version), they act like scattered micro-sponges: they saturate the tissue with moisture, improving its density, elasticity, and that hard-to-fake “inner glow".

This is the logic behind moisturizing and biorevitalizing treatments. They are not about changing the shape of the face or lifting anything — they are about the condition of the skin itself: to make it better hydrated, smoother, less “papery." The effect is subtle and evenly distributed, not concentrated in one spot. After such a treatment, no one will say “you did something" — they will only say “you look good".

It is a high-molecular sugar (more precisely — its derivative), soft and friendly to tissue. A misleading word, a completely different story.

Second role: a framework that holds shape

And now the most interesting moment — the transition from “hydrates" to “builds volume." What changes in the molecule that suddenly it can lift the cheek or give contour to the lips? The key is one technological procedure: cross-linking (in English cross-linking). This is a process in which free chains of hyaluronic acid are intertwined and linked into a dense, three-dimensional network — something like a stiff jelly or elastic framework.

The difference is like between a puddle of water and jelly. Non-cross-linked acid dissolves and is quickly absorbed — ideal for hydration, useless for support. Cross-linked holds its shape: it does not spill sideways, resists gravity, maintains the space into which it was injected. The denser and more tightly woven the network, the more the preparation “holds its shape" and the longer it stays in the tissue before the body breaks it down.

And here the whole riddle from the beginning of the text comes together. When you hear “hyaluronic acid hydrates," it usually refers to the loose form. When you hear “the cheek was modeled with acid," it refers to the cross-linked, dense, supportive form. The same molecule, two completely different tools — and which one it is depends not on marketing, but on the degree of cross-linking and the density of the specific preparation.

For the delicate, mobile area around the eyes or lips, a soft and pliable preparation that does not create lumps is suitable. For rebuilding support on the cheek or jaw — dense and elastic, capable of truly lifting the tissue. It’s a bit like in construction: a different material for plaster, a different one for a load-bearing wall. Therefore, choosing the preparation is a separate decision, not a detail.

The third role, which is rarely mentioned: water as part of the effect

There is one more mechanism that connects both worlds and can be a source of misunderstandings. Since hyaluronic acid attracts water, even in the supportive, modeling version, part of the visible effect comes not from the gel itself, but from the water that this gel draws to itself after injection. The preparation settles in the tissue and for some time “drinks" water from the surroundings, slightly swelling.

This explains two things that often worry people after the first treatment, which are completely natural. First — the effect can mature over the next few days, as the gel only binds water and settles in the tissue; the final appearance is not always the same as on the first day. Second — immediately after injection, the area may be temporarily more tense or slightly swollen precisely because of this water binding, which regulates over time.

💡 That’s why “too much" is rarely the fault of the acid itself. When the effect looks exaggerated, intuition suggests “too much product." More often, however, it’s about accumulation, underestimating how much water the gel will still draw in, or injecting into an area that does not tolerate the load well. This is one of the reasons why a sensible specialist prefers to add more at the next visit rather than overdo it right away — because the acid “grows" with water even after leaving the office.

Superpower that almost nothing else has: reversibility

Here we come to a feature that is truly unique in aesthetic medicine and which is worth understanding before deciding on anything. The effect of hyaluronic acid is reversible. There is a substance — hyaluronidase — an enzyme that can break down the injected gel and shorten its action. To put it figuratively: it’s something like a “rewind button" for this specific material.

This changes the psychology of the entire decision. Most aesthetic methods work “permanently" or only fade over time, without the possibility of quick correction. Hyaluronic acid is different: if the effect is not liked, is asymmetrical, or simply “not right," there is a tool to correct or remove it, instead of waiting months for it to break down on its own. This is a real argument for starting with hyaluronic acid in aesthetics — as the most “forgiving" method.

Reversibility is an asset, not an invitation to carelessness

That the effect can be reversed does not mean that the treatment is trivial or that it can be done “by anyone, because it will dissolve anyway." Administering acid and its possible dissolution are medical procedures — performed only by a doctor who knows anatomy, knows where not to inject what, and can react when something goes wrong. Reversibility is a safety net for the professional, not an excuse for hasty decisions.

💡 Not every “acid" in the office can be reversed in the same way. The reversibility with hyaluronidase applies to hyaluronic acid — and that is an important advantage. But in aesthetics, there are also other fillers and stimulators based on completely different substances, which the same enzyme will not break down. Therefore, the question “is this definitely hyaluronic acid and will it be reversible?" is entirely justified — it’s not being picky, but being aware of what you are deciding on.

What to do with this — that is, how to read your own need

Since one molecule can play such different roles, the practical conclusion is: don’t ask “do I want hyaluronic acid," but “what does my skin need" — and the answer will suggest which form it is about. This distinction organizes the whole topic and saves disappointments “because it was supposed to hydrate, but it turned out that nothing is visible" or vice versa.
